Question # 3: Do you have a form you use when disciplining an employee? Do you usually have a third party present? Are you willing to share your template with us?

By the way, Question #2 was not labelled (my "bad"). That question is: What is your written policy on employee access to their personnel file, both for reading and photocopying?
