Thanks, Bev. Your template cuts right to the point.

Some folk add a statement at the bottom for "Employee comments". If an irate employee refuses to sign the template, attesting that s/he was at the meeting, s/he may choose to write comments. The good news is that, simply by writing on the page, she is documenting for you that she was at the meeting. This helps you demonstrate your "conscientious rescue effort", especially in the face of a contentious employee.

QUESTION #4: What have you done to "counsel a staff member out the door"? This may be some one who just isn't what your program needs; however, s/he isn't performing so badly that you need to write her/him up? This is an opportunity for us to use our emotional intelligence (EQ) for the good of the program.
