Thank you for your form. This naturally raises the question as to what actions necessitate written disciplinary action. Do you use this for all situations? If a teacher who is regularly prompt arrives 10 minutes late with no explanation, do you complete a form like that or just note it in her file? I have a running list in each of my employees file with the date and the infraction next to it. Some infractions are minor and some more serious. I’m trying to determine if I should be using this type of form in all instances.
