IS ONE PARENT'S SIGNATURE ENOUGH?

When in doubt, go for the most thorough approach.
For example, if you think parents are at odds w/ each other, and may claim they don't agree with a policy, you would do well to make sure they both sign off on the policy/practice.
I'm using the term "parents" here. Considering all the varities and constellations of families in our cosmos, "families" may be a wiser term.
Bottom line: get at least one signature, and if you can, make sure that that parent/family member is able to speak for the others responsible for the child.
When in doubt, get all the signatures you need.
