Progressive discipline

We have a form that goes form verbal warning to termination. It includes attendance as well as job performance and conduct/behavior. It also lists previous discipline occurrences with dates and types. A manger's statement includes dates,types,locations as well as actions/statements of employee. Also the appplicable policy must be referenced as well as expectations for improvement. There is a place for employee's statement. Signatures of employee,manager, and HR are required. The following is above the signatures: By signing this notice, I acknowledge that it has been discussed with me, that I have received a copy of it, and that I may submit a written response to become part of the record in my employee file. My signature does not necessarily indicate agreement with the statements presented.
